Nursing units may be added to an Individual Plan of Care (IPC) if the IDT/Service Planning Team identifies a need. A comprehensive nursing assessment that must be conducted by an RN or a focused assessment conducted by an LVN may be justified by the IDT/ Service Planning Team.
