The proof of completion documentation must include certificates or other proof of completion that indicates: 1. the name of the training provider; 2. the course title; 3. course date(s); 4. the hours successfully completed; and 5. the online CE lookup (which has not been updated since September 1, 2005) can be printed and used as acceptable documentation for up to 48 hours of credit (or less for recent hires). Officers should maintain a record of these documents as well as any additional handouts, timelines, brochures, etc. which may aid in the evaluation of training.
