What kind of after-care will be available for patients treated in health centres?
Exactly the same as that available for patients treated in hospital. If aftercare is required it will be provided in the most suitable place according to patient needs. This may be in a specific building (such as a Community Health Centre) or within the patient’s own home. The community matron service will play a key role, since patients are referred to community matrons from acute care and intermediate care to maintain health and manage chronic disease to prevent the patient’s condition(s) worsening.
