• Application form. • Current one-page resume. • Two Reference Forms, at least one from a faculty member (minimum of two – maximum of three). • Participant Agreement Form. • Portfolio. • Copy of your driver’s license if you plan to drive while participating in the program. • Letter of explanation regarding GPA if you do not satisfy the minimum GPA requirement.
