The following must be completed on the MA-10000(S): Item 3 Operational status, Item 5 Total value of products shipped and other receipts, Item 7A – Total employment: first quarter production workers and first quarter other workers, and Item 7B – Total annual payroll before deductions (exclude fringe benefits cost). The following must be completed on the MA-10000(L): Item 3 operational status, Item 5A Total value of products shipped and other receipts, Item 7A Total employment: average annual production workers and first quarter other workers, Item 7B Total annual payroll before deductions (exclude fringe benefits cost), and Item 7C Number of hours worked by production workers. You may be contacted by a Census Bureau representative if other items are missing or incomplete.
