What is warfarin (Coumadin®)?
Warfarin (Coumadin®) is a medication used to prevent blood clots from forming or growing larger. It works by stopping the formation of substances that cause clots. It is often given to patients who have had blood clots or are at risk for developing a blood clot. Why do I need frequent monitoring if I’m on warfarin (Coumadin®)? Patients must take an appropriate dose of warfarin (Coumadin®) in order to prevent clot formation or bleeding. Since many medications, foods, and disease states can affect your response to warfarin (Coumadin®), frequent blood tests and discussions with your doctor or pharmacist are required. REFERRAL PROCESS Do I need a referral from my doctor? If your doctor believes this service is right for you, he/she will refer you to our Anticoagulation Clinic. Your doctor must refer you to our service in order to participate. After receiving your referral authorization, you will be scheduled for your first visit.
