UCR stands for Usual, Customary, and Reasonable Charge. UCR is the amount that is consistent, in the judgment of Harvard Pilgrim/UBH, with the normal range of charges by Healthcare Practitioners for the same, or similar, products or services in the geographical area where the product or services was provided to the Member. The Usual, Customary, and Reasonable Charge is the most that Harvard Pilgrim/UBH will consider in determining allowable amounts for covered benefits.
