What is UCR (Usual, Customary and Reasonable)?
Reasonable and Customary is defined by our health plan as “those charges made for medical services and/or supplies essential to the care of a Covered Person which will be considered reasonable and customary if they are the amount normally charges by the Provider for similar services and supplies and do not exceed the amount ordinarily charged by most providers for comparable services and supplies in the locality where the services or supplies are received.
