DO NOT DESTROY the test booklet. Notify your Principal/Building Level Coordinator and then, transfer the student’s work to another test booklet. Complete a Scribe Verification Form at the conclusion of the transcription. The contaminated booklet must be sent to the County Test Coordinator. He/She may destroy the contaminated test booklet after notifying the Office of Assessment and Accountability by telephone, CTB/McGraw-Hill by letter, and the West Virginia Department of Education, Office of Assessment and Accountability by letter. Any destroyed book must be identified on the Security Checklist (WESTEST Test Coordinator’s Manual, page 26).
