What is the Texas Diabetes Pilot in Medicaid Managed Care?
• Comprehensive diabetes education and Health Care directed by the Primary Care Physician, utilizing the “Minimum Standards for Diabetes Care under Managed Care in Texas”. Emphasis is on diabetes self-management patient education as well as professional diabetes education programs for the Physician.
