What is the team approach?
Each Capital Hospice patient is under the care of a hospice team composed of professionals who can address not only physical but also emotional, social, and spiritual needs. A fundamental tenet of hospice is that emotional and spiritual pain are just as real and in need of attention as physical pain, so your team offers the skills to deal with each. Working closely with you and your loved ones, the team creates an individualized Plan of Care that will be followed by everyone involved in care.
