The adult version of the CAHPS PCMH Survey is designed for patients 18 years and over. The child version is for parents and guardians of children up to age 17. The final survey instruments will be available in both English and Spanish. The CAHPS PCMH Survey can be used for the general population. Specific self-management items in the survey are only applicable to patients with chronic conditions.
