A. An Application for Adjustment of Claim form must be filed with the Illinois Workers’ Compensation Commission within three (3) years of the date of your accident or within two (2) years of the latest payment of compensation, whichever of these dates is later. An Application for Adjustment of Claim form can be filed by either you or your attorney. If the form is not filed with the Illinois Workers’ Compensation Commission within the Statute of Limitations, you will lose all your rights and benefits under the Illinois Workers’ Compensation Act.
