Yes. The Required School Report Supplement is a separate form requested in addition to the Common Application School Report. The information solicited on the School Report Supplement is required and the form must be completed for an application to be considered complete, regardless if a disciplinary report is submitted by an applicants high school. Applicants who do not have this form completed by their high school college advisor/guidance counselor will receive an incomplete notification and will not have their application reviewed.
