Employees are required to submit a full duty medical release from a physician to the AMO 10 days prior to their return to work date. Employees using FMLA for reasons other than their own medical condition are required to submit written notification of intent to return to work. Absence Management will prepare a “Notification of Eligibility to Return to Work” form and forward to the employee’s Human Resources staffing specialist. After providing the appropriate documentation to the AMO, employees will coordinate return to work with their respective Human Resources Staffing Specialist.
