What is the re-entrance health and immunization requirement?
All full-time students entering Old Dominion University must submit a completed Health History Form and provide evidence of having received 2 doses of the MMR vaccine, 1 dose of Meningitis vaccine (or the signed waiver form), 3 doses of Hepatitis B vaccine (or the signed waiver form), and a current Td vaccine booster within the last 10 years. Students may also be required to show proof of a recent PPD Tuberculosis Skin test based on risk factors that are on the Tuberculosis Risk Assessment on the Health History Form. This form must be returned by mail or faxed (757) 683-5930) to Student Health Services by the deadline for the respective entry term. It is not required that you submit the required health history information prior to your PREVIEW date; however, if you are a full-time, degree-seeking student, you should submit the required information as soon as possible.
