What is the purpose of the uniform application?
The uniform application reduces the paperwork burden for practitioners applying for hospital privileges or for participation in a healthplan by eliminating the need to fill out a different application for each hospital and healthplan. Instead, the same application can be submitted to any hospital and healthplan in Georgia that accepts the uniform application. The practitioner can fill out the uniform application and save it in the office computer. When the practitioner wants to apply to a hospital or healthplan, the application can simply be updated if necessary, printed out, signed and dated, and submitted along with the appropriate Part 2.Please insert your text here.
