What is the purpose of the PreMIS Preliminary Form?
The PreMIS Preliminary Form was designed to meet two potential needs. First the form is a resource and training form for those who are learning to use the electronic patient care reporting system known as PreMIS. A second important function of the PreMIS Preliminary Form is to provide a form that can be left with the patient at the destination (hospital, nursing home, etc.) when the formal patient care report has not been completed.
