The questionnaire and research behind the feedback you provide while help you quickly understand areas where you may be at risk of serious medical complications. Identifying those risks early can hopefully lead to early interventions and avoided high cost claims in the future for both you and the company. The questionnaire is also completely confidential and protected by law.
