The Coordinated Healthcare program is based on a “coordinated process of care” that encourages patients to: 1. establish and maintain a relationship with a Primary Care Physician; 2. begin all healthcare events with a call or visit to a PCP; 3. The Coordinated Healthcare Program is intended to provide a staff of Care Coordinators who serve as “advocates” by helping navigate the patient through the often complex process of healthcare. The PCP serves as a “coordinating physician” who can guide and direct the care process among multiple specialists or other healthcare resources that may be required. Since physicians and hospitals do not have the staff or resources to coordinate a patient’s activities outside the treatment room, the Coordinated Healthcare program — working on behalf of your employer health plan — provides a staff of Care Coordinators, who work in support of physicians and their office staff.
