What is the process to request criminal history data on potential employees?
A Non-Law Enforcement Record Check Request form must be submitted to the Iowa Division of Criminal Investigation (DCI). The cost of a criminal history search is $13 per surname or $15 per surname for fax service of pre-established business accounts. The employer must pay the cost of the records check of a potential employee. The request for information will be based upon name, including maiden name and aliases, and date of birth. Because this information is not sufficient to precisely identify a person and the records are based on reports from other criminal justice agencies in Iowa, the completeness and accuracy of the information provided cannot be guaranteed. To obtain a request form, write to DCI, Bureau of Identification, Wallace State Office Building, Des Moines, Iowa 50319 or call (515) 281-5138 with questions.
