If you apply to be an unlicensed provider, you and all persons in your household age thirteen or older must pass a criminal and child protection background check. You must supply your Social Security number or other documentation showing you are legally able to work in this country. Parents wanting you to provide care for their children should request a Legal Unlicensed Provider Registration application from the child care case manager. This must be signed by both the parent and provider. To become a registered legal unlicensed provider you must completely fill out the application, attach all required documentation, and have the necessary signed Release of Information forms so that criminal and child protection records can be obtained. Criminal records and any substantiated complaints of child abuse may be cause for denial of your application to become a legal unlicensed provider.
