A. You need to complete an Application for Coverage form in order to add your dependent. If you are an active employee, you can get a form from your Human Resources office. If you are a COBRA participant, retiree, or surviving spouse, you can contact Blue Cross & Blue Shield at 1-800-709-7881 to get a form. Refer to the Health Insurance Eligibility and Enrollment section of the Plan Document for enrollment timeframes.
