A. You must contact the state board of nursing where you want a license and request an application for licensure. Click here for information to contact other boards of nursing. Complete the Nursys license verification request form and submit to the National Council of State Boards of Nursing with the appropriate fee. The NURSYS verification form can be either downloaded or on-lined from this link. The verification form is also available through the state boards of nursing or National Council. If you have questions regarding this form, please contact the Nursys License Verification Department at 312/525-3780 or toll free at 866/819-1700.
