Please schedule you appointment at least 14 days out, and forward a copy of the appointment slip to the Student Detachment. Your orders will be processed, and sent to you. After you complete your appointment request a document from the clinic which confirms your kept appointment, forward the confirmation document with your 1351-2 and other pertinent paperwork.
