What is the process for obtaining the HPV vaccine?
If you are a woman between the ages of 19 and 26: • Apply for the HPV Voucher at http://depts.washington.edu/hpvshots/. • Once you have obtained the HPV voucher, visit Hall Health and check-in at the Patient Service Center (PSC) (located in the front lobby) and present your HPV voucher to the PSC representative. You will be asked to pay the $35.00 administration fee for each vaccine dose. • Once payment is received, you will be directed to the Immunization Clinic’s waiting area. Wait for an Immunization Clinic Nurse to call your name and receive your first vaccine dose. Because the HPV vaccine is a 3-shot series, you will need to return to Hall Health in two months to receive the second dose of the vaccine and in 6 months to receive the third. Please print a voucher for each subsequent visit and check in with the PSC to pay the administration fee for the dose. You will then be re-directed to the Immunization Clinic.
