What is the process for obtaining Medical Record Collection services from Health Advocate?
The patient contacts e-Cleveland Clinic, which determines if he/she is an appropriate candidate for a second opinion via the Internet. If yes, the patient submits medical history information and answers questions specific to the diagnosis. The patient would then gather information pertinent to the diagnosissuch as pathology slides or X-rays. This is where Health Advocate comes in. e-Cleveland Clinic will provide a Medical Record Check List which outlines the records needed to process the e-second opinion. This list can be shared with Health Advocate, via secure e-mail or secure fax. In addition, an advocate will walk the patient through completing the Authorization for Use and Disclosure of Private Health Information and The e-Cleveland Clinic Proxy Form, which confirms the members desire to use the services of Health Advocate.
