What is the process for forms that are incomplete or illegible?
If the registration/consent form is incomplete or cannot be clearly read, the child will not be able to receive their vaccination on their designated clinic day. On-line registration/consent forms cannot be accepted by Mollen Immunization Clinics unless all required areas are filled in and can be clearly read. If parents need help filling out the registration/consent form, they can call the Mollen Immunization Clinics Client Relations Team at 480-214-2000, ext. 8223.
