You can obtain the Tobacco Status Change Form on HRDirect, under Benefit Forms. Or you can call 1-888-ALCOA123 and speak to a customer service representative. He or she will email you a declaration form. You will need to fill it out and declare that you have been tobacco free for the last 30 days and will continue to be tobacco free. You can return the form by faxing it to 412-553-2822; mailing it to Alcoa Inc, 201 Isabella Street, 4B13, Pittsburgh, PA 15212, or emailing it to IQuitTobacco@alcoa.com. The credit will be applied as soon as administratively possible.
