If minor changes from the original protocol submission become necessary, they should be submitted for IACUC consideration prior to initiating the change. They can be submitted using the online protocol system eProtocol. Information and instructions on how to create an amendment request are available on the IACUC eProtocol Help page. (Protocols approved before the implementation of eProtocol in October 2009 can be e-mailed to the IACUC Coordinator for IACUC review and approval before they are instituted). Be sure to include enough information such that an IACUC review can review the amendment thoroughly-this usually includes a description of the proposed, minor changes, and a rationale or reason for the change. If the change is significant (e.g. no surgery to surgery, terminal surgery to survival surgery, significant increase in number of animals, etc.), a new protocol may be required. If you are unsure whether your change is major or minor, please contact the IACUC Coordinator for help
