The Statutory Agent files a Multiple Agent Change form with box #2 checked, their current name and/or address, the new name/address that they want the records changed to and a check for the initial $125.00. The Secretary of State’s office uses the information on the form to produce a list of the entities for which the customer is listed as Statutory Agent. For each entity on the list there is a box to check if they want that name and/or address changed and a box to check if they do NOT want the name and/or address changed. The Agent then reviews the list, using the check boxes to indicate which entities are to be changed or not changed, signs the report and returns it to Secretary of State’s office with a check for $3.00 per entity to be changed. The second part of the process is then done to change the addresses in the records and produce a report showing all the entities that had the agent name/address altered.
