What is the procedure for registering domiciliary Birth/Death?
Incase of deaths in the house, ·Application form with the signature of five members and the concerned councilors to be given to the Respective Health Inspectors within 20 days. Incase of Accident/Suicide/Murder post mortem report to be enclosed. Incase of births, Authorized family member should fill up the prescribed application form & produce to the respective zonal office within 20 days from the date of Birth.
