What is the procedure for out-of-state patients?
If you live outside of Utah, the procedure is as follows: • Call the clinic at (801) 272-1246 to discuss your needs, provide initial information and set up a time for an initial telephone consultation. • Download and print the new patient paperwork. Fill it out completely, being sure that you indicate the severity of your symptoms by number and not just with a check mark. Then send the completed paperwork by mail to: Utah Weight Loss & Natural Hormone Clinic 2046 Murray-Holladay Road, Suite 204 Holladay, UT 84117-5125 • As with an in-clinic visit, you may need a saliva test kit. If this is the case, we will mail one to you. The test will be performed the first thing in the morning, and again in the evening before bed. You will then send the test in a pre-paid mailer to ZRT Laboratories for analysis. It will take approximately two weeks for these results to come back to the clinic.
