Forward the following documents to the Directorate of Distance Education. Duly filled in application form. The student can download the application form from the university website www.dde.smuhmts.edu. A Demand Draft as prescribed in the application form drawn in favour of Sikkim Manipal University, DE, payable at Manipal/Udupi. The postal address for sending the application is as given below: Controller of Examinations Directorate of Distance Education, 1st Floor, Corporation Bank Building, Sikkim Manipal University of Health Medical & Technological Sciences, Manipal – 576104.
