He or she may be able to help you right away. You may also call the toll free Customer Service number on your member ID Card. If you are still unhappy and wish to file a complaint, you should fill out a “Member Issue Form”. You can get this form from your Anthem Blue Cross HMO coordinator or from us. You may also call us at the Customer Service number on your member ID Card and ask one of our Customer Service Representatives to fill out the Member Issue Form for you.
