What is the procedure for completing school/camp forms and referrals (updated 05/25/10)?
With rare exception, we are able to complete school/camp forms and referrals within 3 business days of receiving the request. To increase the accuracy and expedite completion, please complete the “parent” section of all forms prior to the request. Unfortunately, we are not able to complete same day requests for forms and referrals. Therefore, please ensure that we receive your form or referral request at least 3 days before it is due. Once they are completed, we are happy to mail, fax, or e-mail to you the forms and referrals. Please note: To ensure the forms are accurate and complete, your child must have a well-child exam with Hirsch Pediatrics within 1 year of completing the forms. Please note that we are not able to complete the forms if your child has only had sick visits at the office or has not had a well-child exam at Hirsch Pediatrics within 1 year of completing the forms.
