What is the Post-Service Survey?
Solution: The Post-service survey is a data collection instrument that was developed and field-tested by the American Foundation for the Blind (AFB) with financial support from the National Institute on Disability and Rehabilitation Research (NIDRR). The Post-service survey form is used to gather demographic data about clients who have received Independent Living Services under Title VII-2 of the Rehab Act. Responses to the questions presented on the Post-service form can be compared to the responses submitted on the Pre-service form (see Pre-Service Survey FAQs for help) and, when used in tandem with the outcome data provided on the Pre/Post-service Independent Living Functional Assessment (ILFA) forms (see Pre/Post-Service ILFA FAQs for help), may be used as the basis for your state or agency’s program evaluation study.
