What is the physics departments policy on use of the Kent State Letter of Recommendation form?
If your recommender plans to send a paper letter, he/she is free to add this form or not, whichever is preferred by you and/or the recommender. Recommenders who send an e-letter rarely, if ever, include the form. However, please note that we request your recommenders not to send the form as a substitute for a letter.
