What is the Payer/Carrier Acknowledgement form?
Included in your initial enrollment packet, this form is extremely important. Be sure to fill it out, especially if you have been submitting claims electronically to payers before signing up for Per-Se s clearinghouse services. It helps us coordinate with each payer the actual date you will stop using your current submitter ID and begin using your new Per-Se submitter ID. Without this coordination, your ability to send electronic claims to these payers could be interrupted.
