What is the Partnership for Chronic Disease Prevention and Management Project in Western Wisconsin?
This grant project is an academic and community partnership between Viterbo University, St. Clare Health Mission, Medical College of Wisconsin, and La Crosse, Monroe, Trempealeau, and Vernon counties. The partnership project is funded by the Healthier Wisconsin Partnership Program, a component of the Advancing a Healthier Wisconsin endowment at the Medical College of Wisconsin. Viterbo University received $329,349 over a three year period, 7/1/08 through 6/30/11, to develop new health screening and education services for uninsured and underinsured citizens in each of the four counties: La Crosse, Monroe, Trempealeau, and Vernon. The grant project staff is housed at Viterbo University in Brophy Nursing Center Room 113. MISSION: The project seeks to impact morbidity and mortality from chronic disease through increased prevention and health promotion activities for underserved communities in Western Wisconsin.
