What is the Nursing Supplementary Application?
It is a form that is required for a complete application to the 2nd Career Program. It lists the prerequisites to our program. You will fill out the term you completed each one, what the course name and number was, the number of credits of each course, the school where you took it, and the grade you received. If you haven’t taken the prerequisite(s) when you fill out this form, please indicate when and where you plan to take the course(s).
