What is the NPDB/HIPDB self-query report and how do I complete it?
A. The NPDB/HIPDB is the National Practitioner Data Bank/ Healthcare Integrity and Protection Data Bank. A self-query is required of all applicants applying for a full medical/osteopathic license. Please go to the following website: http://www.npdb-hipdb.com/, choose ‘Perform a Self-Query’ in the Quick List on the right.
