A. The NPDB/HIPDB is the National Practitioner Data Bank/ Healthcare Integrity and Protection Data Bank. A self-query is required of all applicants applying for a full medical/osteopathic license. Please go to the following website: http://www.npdb-hipdb.com/, choose ‘Perform a Self-Query’ in the Quick List on the right. You will need to mail the completed self-query report to the Board once you receive it from the National Practitioner Data Bank. Please also refer to the application instructions, page 4 #12, for how to complete the self-query.
