What is the new Parent/Guardian Consent Form?
Beginning in 2010-11, this form is required for all entries by National PTA. The form is required to be signed by the parent of each child under the age of 18 years who appears in, or is heard in, a Reflections entry in any category, even if the child is not the creator of the artwork. Entries arriving at the State level without consent forms may be disqualified.
