The network for those participating for these plans is Humana/ChoiceCare PPO. In addition, a second tier has been created to include the UK HealthCare network. This network includes the UK Chandler and Good Samaritan Hospitals, KY Clinics and participating Urgent Treatment Centers. Certain services will be provided at reduced copayments. Remember, the UK-PPO plans offer out-of-network benefits, and the UK-EPO network does not. The networks may be accessed on Humana’s website: www.humana.com.
