The state requires that the following items be provided along with verification of passing the learners permit test. If your student is age 15 when they start the class, we request that these items be brought to parent orientation but no later than the 3rd day of the class. If they will turn age 15 after the 3rd day of class, we do not require this until they reach that age. o Birth Certificate (original or certified copy) ** o Social Security Card (original card) ** o Verification of Enrollment form (TEA form) from students school ** o DL-14A Application for Drivers License (provided by Green Light Driving Academy) o $15.00 permit processing fee ** If not available, substitutions can be provided. For more information about substitutions, please contact Green Light Driving Academy. PLEASE NOTE: If your student is not a U.S. citizen, additional documentation may be required to obtain the learners permit.
