What is the Multiple Statutory Agent Change form for?
The form has two purposes. If a Statutory Agent moves and/or changes their name, they can use one form to change their name and/or address altered on all the entities for which they are the agent. Also, if they have the same address in our records in multiple versions, they can use this form to standardize all the variations into one version.
