The information is required for first-time freshmen only! Please read this information. In accordance with California Health and Safety Code Section 120395, all first-time freshmen who will be residing in on-campus housing must acknowledge receipt of this information by completing and submitting this data online. This form is for informational purposes only. No immunization is required for you to live in the residence halls. You must answer the two questions at the end of the information page regarding Meningococcal Disease and Immunization. Once you have completed and submitted this data online it will be sent electronically to OHA. It will show as “Submitted” on your WebPortal in the status details until your License Agreement documents and $800 initial payment have been received and processed by OHA.
