The format for the List of Potential Eligible voters provided below must be followed. Specifically, the format of the List of Potential Eligible Voters must be prepared in five columns or fields exactly as shown in the sample format in the docketing letter (columns A, B, C, D, E, only). There must not be any other information or data in the file, diskette, or CD, except as displayed in the five columns or fields on the sample format.
