What is the Institutional Nomination Form?
The Institutional Nomination Form (see below) officially nominates a candidate for the award. An orginal, signed hard copy of this form must be submitted to BWF by the application deadline. The Official responsible for sponsored programs at your institution must sign this form. This should be the same person you entered as the “Signing Official” on the Nominating Institution & Contacts screen.
