The IVF process is the issuance of the Digital Certificate. • You will receive an e-mail explaining the IVF process. This e-mail also contains a link to an IVF form for you to complete. The form must be signed, notarized and returned to VisionShare within 30 days. • You will receive a hard copy letter by regular mail that will contain your PIN number for your Digital Certificate. • You will receive an e-mail with Digital Certificate attached. The attachment is a file that ends in .p12. • Using the PIN from the hard copy letter, install your Digital Certificate using the instructions found in the e-mail. To expedite the process you may fax the IVF to VisionShare, but make sure that you also mail the original to VisionShare within 30 days. Please note: If VisionShare does not receive the original, notarized IVF within 30 days, your Digital Certificate will be deactivated and you will not be able to proceed using EDISS Total OnBoarding.
